Addressing Social Determinants of Health: Housing as the Foundation for Well-Being
Horizon’s Housing Program takes a “no wrong door approach” that allows members to access housing services and supports through multiple referral points. Horizon’s team of Housing Specialists are a part of a multidisciplinary approach in ensuring members and families can access safe, stable, and affordable housing. Housing Specialists act as liaisons between members, housing providers, and support services, offering expertise and guidance to navigate complex housing systems. They have established wide-ranging partnerships with community resources and advocacy agencies throughout NJ. Horizon’s Housing Specialists provide an integrated service delivery approach that addresses pre-tenancy, tenancy, and long-term sustainability. By addressing needs at each stage, Housing Specialists ensure members and families ultimately reduce housing instability and homelessness which in turn improves health outcomes.

Beyond Clinical Care: Implementing Integrated Strategies to Address Social Determinants of Health Across the Care Continuum
According to the World Health Organization, social determinants of health account for between 30% - 55% of health outcomes, highlighting the importances of integrated strategies for addressing unmet needs while delivering health care across the continuum. This session expands on the compelling data on the impact of social factors on health outcomes, healthcare utilization and cost while sharing integrated strategies for addressing SDOH needs from universally screening for unmet needs, training and skill building for the entire health care team to address unmet needs regardless of the point of entry and promotion of patient empowerment and self-efficacy to increase individuals’ capacity to address unmet needs in the future.

Medicare’s New Health Equity Services: Lessons Learned from a Multi-Market Implementation Cohort
CMS finalized Medicare Part B services called SDOH Risk Assessment, Community Health Integration, and Principal Illness Navigation in the 2024 Physician Fee Schedule. CommonSpirit Health, a non-profit health system with a presence in 24 states, launched a 16-month pilot in September 2024 to test implementation of these services across 7 markets in order to better sustain auxiliary personnel positions such as Community Health Workers. Cohort participants include a cross-mix of use cases, including an MSSP ACO, rural care delivery sites, a medical group contracting with a local community-based organization, and a 501c3 organization that is the primary social care provider of aging services across multiple rural counties and a subsidiary of a CommonSpirit hospital. This session will share emerging lessons from the implementation cohort, including the process and structure supporting the cohort, as well as initial outcomes.

Workshop: Food Is Medicine
This interactive session will demonstrate how a collaborative approach to delivering a Food is Medicine (FiM) pilot program improved clinical outcomes, increased nutrition literacy, and enhanced overall well-being. Facilitators will cover assessing social and clinical needs, and guide participants through program design, implementation, and sustainability phases. Participants will receive guidance on how to develop a customized strategic plan for advancing Food Is Medicine initiatives, supported by frameworks, tools, resources, and lessons learned from Huron’s experience managing a successful FiM pilot program.
